STORY
Pharos Innovations was founded in 1996 by Dr. Randall Williams, a Johns Hopkins trained physician, with the belief that engaging patients in their daily self-care is the optimal path to reducing medical cost. In response, Pharos developed Tel-Assurance®, a real-time patient engagement and monitoring technology platform that drives daily behavior change and provides early warning of impending care problems so that providers can efficiently and cost-effectively manage their "at risk" populations.
From 1996-2004, Pharos undertook extensive clinical validation testing and refinement of the Tel-Assurance platform, adding multiple condition modules, population risk screeners, reporting and analytics capabilities, and refinement of the system's algorithms. In addition, Pharos assists clients with care model design, analytically driven population targeting, care coordinator workflow optimization, and large scale population onboarding into targeted population management interventions leveraging our extensive analytic and clinical design expertise.
Since its commercial launch in 2004, Pharos has continued to develop, refine, improve and validate the impact of Tel-Assurance on such populations as Medicare, Medicaid, Commercial, and the Veteran's Administration. Over the past 8 years, Pharos has successfully implemented Tel-Assurance in several large scale clinical trials, market-wide payer initiatives and CMS demonstrations.
Large scale and multi-site implementations have allowed us to understand the complexities of a broad range of organizational models, and to refine our approaches to implementation, inluding refined population targeting, clinical intervention design and care manager workflow optimization, and allowing us to adapt our approach to meet the unique needs of populations and client organizations.
