Our vision is to transform the health of populations and to heal a broken healthcare system through scalable, data-driven, and cost effective solutions that are grounded in deep clinical and healthcare domain expertise and care innovation experience.

We believe that the time has come in healthcare for a new model of care: one that engages consumers to become active participants in their care.  We envision healthcare that places a premium on a system of care that provides the tools and techniques to individuals and families to take better care of themselves; one that leverages the best of process design, data and analytic insights, and consumer friendly communication technologies to build a highly effective, scalable and economically sustainable system, where the patient is at the center.

MISSION

Our mission is to improve the lives of those most at risk of care complications and avoidable hospitalizations.  Typically these are individuals with chronic conditions or patients in a transition of care from one setting to another, who normally are left to "fall through the cracks" of a healthcare system that is poorly coordinated and too often not patient centered.

We accomplish this by creating a daily connection between patients and their care providers.  Our customers' care providers have visibility on large populations each day with direct line of sight to those individuals that may be at risk of an unnecessary readmission.  Armed with this actionable information, clients are able to better coordinate care for their patients, reduce hospitalizations and practice in a more cost-efficient manner.
